The Coahoma Community College (CCC) Sports Medicine Staff strives to provide every student athlete with the most comprehensive and highest quality healthcare within our means. The primary goal of CCC Sports Medicine is to create a positive recovery experience for a safe return to full athletic participation. At all times, the overall welfare of the student-athlete will be our top priority.

Prior to any sports participation at Coahoma Community College student athletes must complete and submit the following documents and items to Selina Reid, who is CCC's Assistant Athletic Director/Head Athletic Trainer:
 
  1. Download HEALTHY ROSTER APP on their electronic device(s), and complete required information within Healthy Roster.
  2. Current PRE-PARTICIPATION PHYSICAL EXAM FORM A must be used and healthcare provider must submit business card or healthcare facility name stamped on it to be valid, if currently under a physician care must submit written medical clearance and medical records.  If prescribed MEDICAL EMERGENCY PRESCRIPTIONS (such as asthma inhalers, EpiPen, etc. must be available onsite, prior to any sports participation. 
  3.  Complete CCC STUDENT ATHLETE SPORTS MEDICINE FORM B.
  4. FRONT & BACK COPY OF MEDICAL INSURANCE CARD(S), if NO available medical insurance complete STATEMENT OF NO OTHER INSURANCE FORM C.
  5. Read and sign SICKLE CELL TRAIT FORM D.
  6. Read and sign DRUG TESTING POLICY FORM E.
  7. Read and sign CONCUSSION MANAGEMENT ACKNOWLEDGEMENT OF RECEIPT FORM F.
  8. Complete CONCUSSION BASELINE TESTING by scheduling appointment, please call 662-483-0610 or email sreid@coahomacc.edu.
